Insurance

Sumanas Counseling accepts:

  • Premera
  • LifeWise
  • BlueCross BlueShield
Please verify your benefits. Insurance coverage varies by plan, network, service type, diagnosis, deductible, and benefit design. Acceptance of an insurance company does not guarantee that a specific plan or service is covered. Clients should verify benefits directly with the insurer, and the practice will also complete eligibility verification before confirming coverage.

Questions To Ask Your Insurer

  • Is Avani Vimawala, or Sumanas Counseling in network for my specific plan?
  • Do I have outpatient behavioral health benefits?
  • What is my deductible, and how much has been met?
  • What is my copay or coinsurance?
  • Is telehealth covered?
  • Is family therapy covered, and under what conditions?
  • Is prior authorization required?

Payment

Payment is due at the time of service unless other arrangements have been made. Clients using insurance remain responsible for deductibles, copays, coinsurance, non-covered services, and any balance determined by the insurer.

Cancellation and No-Show Policy

Your appointment time is yours.

  • Please provide at least 48 hours’ notice to cancel or reschedule.
  • Cancellations made with less than 48 hours’ notice and missed appointments will incur a $100 fee.
  • Insurance does not cover cancellation or no-show fees; you are responsible for this charge.
  • Arriving more than 15 minutes late may be considered a late cancellation and may result in a $100 fee.
  • Exceptions for emergencies may be considered on a case-by-case basis.
  • Repeated cancellations or missed appointments may affect your ability to reserve a recurring appointment time.

By scheduling an appointment, you acknowledge and agree to this policy.

Good Faith Estimate

Under federal law, clients who are uninsured or who choose not to use their commercial health insurance have the right to receive a Good Faith Estimate explaining the expected cost of their care.

You may request a Good Faith Estimate before scheduling services. If you receive a bill that is at least $400 more than your estimate, you may have the right to dispute the charge.
